Registration

Also known as the Medical Release Form



As of Monday, November 26, 2012, Cal South introduced a new online Release and Transfer process. The downloadable paper version will no longer be used. Full information about the new process will be released to our full membership by email and on our homepage. For youth eligibility clearance only. It allows a youth player to continue to play for his/her current team while participating in senior trial games. Please see form for further details.



Request for a medical play down exception to allow a Recreational player to play down in age one year if the certain criteria are met for one seasonal year.


Request for player to participate on team aged three (3) or more years beyond player's proper birth date age division.



All players age 18 and over participating in affiliated leagues, who are not eligible for the Prior to 12 or First Registration option, will be required to complete the ITC Request Form (ITC 5-11).




Insurance

In case of an injury during a Cal South sanctioned event, including league play and practice. The claim form must be e-mailed or faxed directly to AIG within 90 days of the injury.

A copy must be faxed or mailed to the Corporate Office in case of an injury during a Cal South sanctioned event, including League play, within 90 days of the injury.
